This web site was developed with support from the Iowa Real
Choices project, which is funded by a U.S. Department of Health and
Human Services Real Choices Systems Change Grant from the Centers
for Medicare and Medicaid Services (CMS). The lead agency for this
grant was the Iowa Department of Human Services; DHS
subcontracted with the Center for Disabilities and Development,
University of Iowa Health Care, to administer the grant.
